Job Description
Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) to provide skilled hospice care to patients in Central Ohio. The RN will be responsible for delivering high-quality nursing care, coordinating with interdisciplinary teams, and supporting patients and families through individualized care plans.
Key Responsibilities
- Perform comprehensive nursing assessments and develop individualized care plans
- Provide direct patient care in accordance with physician orders and hospice standards
- Administer medications and treatments as prescribed
- Monitor patient conditions and update care plans as needed
- Collaborate with physicians, social workers, aides, and other healthcare professionals
- Educate patients and families on disease progression, symptom management, and care expectations
- Maintain accurate and timely documentation in the EMR system
- Participate in the on-call rotation, including every other weekend
Qualifications
- Active Ohio Registered Nurse (RN) license
- Minimum of 1 year RN experience preferred
- Hospice or home health experience strongly preferred
- Experience with Consolo EMR is highly preferred
- Strong assessment, communication, and documentation skills
- Ability to work independently and manage a caseload effectively
- Valid driver’s license and reliable transportation
