Job Description

Job Title: Director of Nursing – Home Health Location: Indiana (Frequent travel to patient homes required) Salary: $95,000/year (increased to $115,000/year after Home Health division launch) Position Summary: The Director of Nursing will play a critical role in the startup and oversight of the Home Health division for Kaiser. This position requires a skilled Registered Nurse (RN) with experience in home health nursing and supervisory experience to ensure safe, effective, and compassionate patient care. Key Responsibilities: Patient Care: -Provide professional nursing services in line with the State Nurse Practice Act, agency policy, and professional standards of practice. -Conduct comprehensive assessments of client status, including physical, psychosocial, and environmental factors. -Develop and maintain individualized Plans of Care (POC) for each client. -Provide skilled interventions to achieve client outcomes within a specified time. -Report significant changes in client status to the physicians and interdisciplinary care team. -Ensure all client documentation, including assessments, interventions, and care plans, is accurate, complete, and in compliance with agency standards. Supervision & Team Management: -Oversee and manage home health care teams and ensure effective delivery of patient care. -Supervise and provide guidance to team members, ensuring compliance with safety and care standards. -Participate in case conferences, in-service training, and performance improvement activities. Compliance & Quality Improvement: -Ensure adherence to all relevant regulations, including Medicare/Medicaid, labor laws, and infection control practices. -Take proactive steps to improve the quality of care and ensure patient safety. -Maintain confidentiality of patient records and all operational aspects of the job. Travel & Availability: -Travel to patient homes is frequent and may require occasional weekend or evening shifts depending on patient needs. -May be called upon to assist in addressing urgent situations or to provide education/training for staff. Qualifications: Education: -Bachelor’s of Science in Nursing (BSN) or Master’s of Science in Nursing (MSN) preferred. -Graduate from an accredited nursing school. Experience: -Minimum of 3-5 years of nursing experience, with at least 2 years in home health care. -At least 1 year of supervisory or leadership experience in a home health setting. Licenses/Certifications: -Valid and current RN license in the state of Indiana. -Valid driver’s license with active automobile insurance. -Current CPR certification. Skills & Abilities: -Strong clinical judgment and critical thinking skills. -Excellent interpersonal, communication, and organizational skills. -Experience with Electronic Medical Records (EMR) preferred. -Demonstrated ability to manage a team and improve patient care outcomes. Other Requirements: -Must complete annual continuing education hours on topics such as Medicare/Medicaid regulations, management practices, ethics, and quality improvement. -Must adhere to all HIPAA and safety protocols. -Must have a clean driving record and be comfortable traveling to patients’ homes as part of daily duties.
