Job Description
Job Title: Hospice Registered Nurse (RN) – Part-Time
Location: Moraine, OH (surrounding areas based on patient needs)
Position Overview
We are seeking a compassionate and experienced Hospice Registered Nurse (RN) to provide high-quality, patient-centered care to individuals receiving end-of-life support. This role involves traveling to patient locations, including private homes, assisted living facilities, and skilled nursing facilities.
This is a part-time opportunity with the potential to transition into a full-time role based on business needs and performance.
Schedule & Compensation
- Schedule: Monday, Wednesday & Friday | 8:00 AM – 4:30 PM
- On-Call: Every other week and weekend (as needed)
- Pay: $35–$40/hour (based on experience)
- Mileage reimbursement provided
Key Responsibilities
- Provide clinical nursing care in accordance with individualized patient care plans
- Conduct initial and ongoing patient assessments
- Develop and update plans of care in collaboration with physicians and interdisciplinary teams
- Ensure compliance with state regulations and payer requirements
- Deliver compassionate hospice care across various settings (home, inpatient, assisted living, SNF)
- Maintain accurate and timely documentation
Required Qualifications
- Active RN license in the state of practice
- Current CPR certification
- Valid driver’s license, auto insurance, and reliable transportation
- Ability to travel to patient locations
- Strong assessment, communication, and critical-thinking skills
Preferred Qualifications
- Previous hospice or home health experience
- Experience with Home Care Home Base (HCHB) EMR is a plus
