Licensed Practical Nurse (LPN) – PRN Rehabilitation
Location: Louisville, KY
Pay: $38.00/hour
Position Type: PRN | One-Year Assignment

Schedule Information
- Day Shift: 6:00 AM–6:00 PM
- Night Shift: 6:00 PM–6:00 AM
- PRN scheduling based on facility needs and candidate availability
LPN Requirements
- Active Kentucky LPN license
- Minimum of two years of nursing experience
- Previous inpatient rehabilitation experience preferred
- Experience administering medications and treatments within the LPN scope of practice
- Strong patient-monitoring and clinical-documentation skills
- Ability to recognize and report changes in patient condition
- Effective communication and interdisciplinary teamwork skills
- Compassionate and patient-focused approach to rehabilitation care
Key Responsibilities
- Provide LPN nursing care to rehabilitation patients during assigned shifts.
- Administer medications and prescribed treatments within the LPN scope of practice.
- Monitor patients and report changes in condition to the appropriate clinical staff.
- Assist with individualized care plans and rehabilitation goals.
- Support patient mobility and activities of daily living as appropriate.
- Work closely with RNs, therapists, and other members of the rehabilitation team.
- Maintain accurate and timely clinical documentation.
- Follow all patient-safety, infection-control, and facility procedures.
- Perform additional nursing duties within the LPN scope of practice.
