Licensed Practical Nurse (LPN) – Acute Rehabilitation – Days
Location: Louisville, KY
Pay: $45.00/hour

Schedule Information
- Day shift
- 8:00 AM–5:00 PM
Acute Rehabilitation LPN Requirements
- Active LPN license applicable to Kentucky
- Minimum of one year of recent nursing experience
- Acute-care or inpatient rehabilitation experience preferred
- Experience administering medications and treatments within the LPN scope of practice
- Strong patient-monitoring and clinical-documentation skills
- Ability to recognize and report changes in patient condition
- Experience assisting with nursing care plans and rehabilitation goals
- Effective communication and interdisciplinary teamwork skills
- Compassionate and patient-focused approach to rehabilitation care
- Ability to follow healthcare regulations, safety standards, and facility procedures
Key Responsibilities
- Provide LPN nursing care to acute-rehabilitation patients during the day shift.
- Administer medications and prescribed treatments within the LPN scope of practice.
- Monitor patients and promptly report changes in condition to the RN.
- Assist with individualized nursing care plans and rehabilitation goals.
- Support patients with mobility and activities of daily living as appropriate.
- Assist with patient admissions, transfers, and discharges.
- Work closely with RNs, physicians, therapists, and other members of the rehabilitation team.
- Educate patients and families regarding care and recovery as directed.
- Maintain accurate and timely clinical documentation.
- Follow all medication-safety, infection-control, patient-safety, and facility procedures.
- Perform additional nursing duties within the LPN scope of practice.

About the Acute Rehabilitation LPN Role
