RN Case Manager Requirements
- Active RN license applicable to Kentucky
- Current BLS certification
- Minimum of one year of recent RN case-management or acute-care coordination experience
- Knowledge of hospitalization and discharge-planning processes
- Understanding of utilization review and appropriate levels of care
- Ability to identify and resolve barriers to discharge
- Experience coordinating post-acute services and community resources
- Strong clinical-review and documentation skills
- Effective communication with patients, families, payers, providers, and clinical staff
- Strong organization, critical-thinking, and problem-solving skills
Key Responsibilities
- Coordinate and manage patient care throughout hospitalization.
- Review clinical information, treatment plans, and patient needs.
- Collaborate with physicians, nurses, therapists, social services, payers, and outside providers.
- Facilitate the appropriate level of care and support utilization-review activities.
- Develop and coordinate safe, timely discharge plans.
- Identify barriers that may delay discharge or affect continuity of care.
- Arrange post-acute services, equipment, referrals, and community resources.
- Communicate care plans and discharge information to patients and families.
- Support care-coordination activities throughout the patient’s hospitalization.
- Maintain accurate and timely case-management documentation.
- Communicate patient updates and discharge needs to the interdisciplinary care team.
- Perform additional case-management duties as assigned.
