LPN Outreach Nurse
Location: Louisville, KY
Pay: $40.00/hour
Pay: $40.00/hour
Schedule Information
- Monday–Friday
- 8:00 AM–5:00 PM
- Day shift
LPN Outreach Nurse Requirements
- Active LPN license applicable to Kentucky
- Experience collecting blood samples and handling clinical specimens
- Previous nursing, community health, home health, outreach, or case-management experience preferred
- Ability to travel to patients’ homes throughout the assigned service area
- Valid driver’s license and reliable transportation
- Strong patient-education and communication skills
- Ability to work independently in community and home-based settings
- Strong organizational, case-management, and follow-up skills
- Ability to maintain accurate and confidential clinical documentation
- Compassionate, professional, and patient-focused approach to care
Key Responsibilities
- Support patients and families through the LEAD program.
- Conduct scheduled home visits throughout the assigned service area.
- Collect blood samples according to clinical and safety procedures.
- Label, document, store, and transport specimens appropriately.
- Provide patients and families with program-related health education.
- Coordinate patient services and complete case-management activities.
- Follow up with patients regarding appointments, testing, care needs, and available resources.
- Communicate patient updates and concerns to the appropriate healthcare professionals.
- Maintain accurate and timely clinical and case-management documentation.
- Protect patient privacy and follow all confidentiality requirements.
- Follow infection-control, specimen-handling, and patient-safety procedures.
- Perform additional outreach nursing duties as assigned.
