Occupational Therapist – PRN
New Albany, IN
Pay: $65/hour

Schedule Information
- Monday–Friday
- Hours scheduled between 8:00 AM and approximately 4:30–5:00 PM
- Up to 10–12 hours per week
- Weekly hours may vary based on patient and operational needs
Occupational Therapist Requirements
- Active Occupational Therapist license valid in Indiana
- Degree from an accredited occupational therapy program
- Education and professional license verification required
- Previous home-health occupational therapy experience required
- Previous OASIS documentation experience required
- Strong patient-evaluation and treatment-planning skills
- Accurate clinical documentation and recordkeeping abilities
- Effective communication and interdisciplinary teamwork skills
- Ability to work independently in patients’ homes
- Compassionate and patient-centered approach to care
Key Responsibilities
- Complete occupational therapy evaluations in patients’ homes.
- Assess functional abilities, safety risks, and rehabilitation needs.
- Develop and implement individualized occupational therapy plans of care.
- Provide therapeutic activities to improve daily living skills, mobility, strength, coordination, and independence.
- Educate patients and caregivers about treatment plans, adaptive techniques, and home-safety practices.
- Complete OASIS assessments and other required home-health documentation.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team.
- Support safe discharge planning and continuity of care.
- Maintain accurate and timely clinical records.
- Follow all patient-safety, infection-control, and agency procedures.
- Perform additional occupational therapy duties as assigned.
