Physical Therapist – PRN

Pay: $65.00/hour

Schedule Information
- Monday–Friday
- Visits scheduled during operating hours of 8:00 AM–approximately 4:30–5:00 PM
- Up to 10–12 hours per week
- Weekly hours may vary based on patient needs
Physical Therapist Requirements
- Current Indiana Physical Therapist license in good standing
- Graduate of an accredited physical therapy program
- Previous home-health physical therapy experience required
- Previous OASIS documentation experience required
- Strong clinical-assessment and treatment-planning skills
- Accurate and timely clinical documentation abilities
- Knowledge of Medicare and home-health requirements
- Excellent communication and interpersonal skills
- Ability to work independently and manage a home-based caseload
- Reliable transportation and ability to travel to patients’ homes
- Compassionate, patient-centered approach to care
Key Responsibilities
- Perform comprehensive physical therapy evaluations in patients’ homes.
- Assess mobility, strength, balance, endurance, safety, and functional limitations.
- Develop and implement individualized plans of care.
- Provide skilled therapeutic interventions to improve strength, mobility, balance, endurance, and independence.
- Conduct gait, transfer, and functional-mobility training.
- Provide fall-prevention education and interventions.
- Evaluate home environments and recommend appropriate safety modifications.
- Educate patients and caregivers about therapeutic exercises and safe mobility techniques.
- Complete OASIS assessments and required home-health documentation accurately and on time.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Communicate with physicians, nurses, occupational therapists, and other members of the interdisciplinary team.
- Participate in care coordination and discharge planning.
- Provide care consistent with Medicare, home-health, and agency requirements.
- Perform additional physical therapy duties as assigned.
