Licensed Practical Nurse (LPN) – Memory Care (Per Diem)
Louisville, KY
Pay: $27.50/hour
Per Diem | Evening Shift
Schedule Information
- 3:00 PM – 11:00 PM
- Per Diem
Licensed Practical Nurse Requirements
- Current Kentucky LPN license required
- Previous memory care, long-term care, assisted living, or skilled nursing experience preferred
- Experience caring for patients with Alzheimer's disease, dementia, or cognitive impairments preferred
- Current CPR/BLS certification required
- Strong medication administration, patient assessment, and clinical documentation skills
- Excellent communication, compassion, and interpersonal skills
- Ability to remain calm and responsive in changing patient situations
- Commitment to providing person-centered, compassionate care
Key Responsibilities
- Provide direct nursing care to residents with Alzheimer's disease, dementia, and other memory-related conditions
- Administer medications, treatments, and therapies while monitoring residents for side effects and changes in condition
- Perform ongoing physical and cognitive assessments and communicate concerns to physicians and the interdisciplinary care team
- Assist in developing and updating individualized care plans based on each resident's needs
- Promote a safe environment by monitoring for fall risks, wandering behaviors, and other safety concerns
- Supervise and support nursing assistants while providing guidance on dementia-focused care techniques
- Educate residents' families on disease progression, care plans, and available support resources
- Maintain accurate clinical documentation and ensure compliance with facility policies and state regulations
- Deliver compassionate, patient-centered care that promotes dignity, comfort, and quality of life
