Acute Rehab Licensed Practical Nurse (LPN)
Louisville, KY
Position Type: 13-Week Contract | Nights
Schedule Information
- 13-week contract
- Night Shift
- 6:30 PM – 7:00 AM
Acute Rehab LPN Requirements
- Graduate of an accredited Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program
- Current Kentucky LPN license required
- Current CPR/BCLS certification required
- ACLS certification preferred
- IV Certification required or obtained within six months (highly preferred)
- Minimum of six months of Medical/Surgical experience in an acute care setting preferred
- Excellent verbal, written, and interpersonal communication skills
- Ability to provide compassionate, patient-centered care in a rehabilitation setting
- Strong organizational skills and ability to work effectively in a fast-paced environment
- Ability to remain flexible and perform well under pressure
Key Responsibilities
- Provide direct patient care under the supervision of a Registered Nurse
- Assist with patient assessments and reassessments while identifying appropriate nursing interventions
- Administer medications safely and accurately while following established medication procedures
- Deliver patient care based on individual treatment plans and rehabilitation goals
- Monitor patient conditions and communicate changes to Registered Nurses, physicians, and the interdisciplinary care team
- Educate patients and families on treatment plans, rehabilitation goals, and pain management techniques
- Perform treatments and nursing services within the scope of LPN licensure
- Complete timely and accurate patient documentation and quality improvement assessments
- Participate in interdisciplinary care planning and team conferences
- Promote patient safety while maintaining compliance with HIPAA, hospital policies, and quality standards
- Maintain a clean, safe, and professional healthcare environment
- Support Continuous Quality and Performance Improvement (CQPI) initiatives
- Perform additional duties as assigned
